HYPERTENSIVE RETINOPATHY

 What is Hypertensive retinopathy?

           Hypertensive retinopathy is the damage to retinal vessels and retina due to high blood pressure i.e. hypertension. Early this is asymptomatic after that retinal vein occlusion starts.



What is the difference between Hypertensive retinopathy and Diabetic retinopathy? 


Diabetic retinopathy

Pathogenesis

  • Precapillary arterioles
  • Capillary
  • Venules

The microangiopathy has following;

  • Capillaries leakage
  • Capillaries occlusion

Symptoms

  • Impairment of vision
  • early-stage asymptomatic
  • Involvement of macula causes gradual loss of vision
  • Sudden loss of vision due to vitreous hemorrhages

Signs

  • Background of Diabetic retinopathy
  • Pre-Proliferative Diabetic retinopathy
  • Proliferative Diabetic retinopathy
  • Diabetic maculopathy

 

  Hypertensive retinopathy

Pathogenesis

  • Vasoconstriction narrowing of arterioles
  • Increase in vascular permeability
  • Arteriosclerotic changes  ( thicking of vessel walls) 
Symptoms

  • Hypertension is asymptomatic
  • Sudden onset of painless vision loss.
Signs

  • Cotton-wool spot
  • Optic disc edema 
  • Hemorrhages
  • A-V nicking
  • Flamed shape hemorrhages



 What is the treatment of Hypertensive retinopathy?

 With the control of systemic hypertension, the retinal lesion may resolve with time.
